Why is Medication Therapy Management (MTM) important for me?

If you use prescription medications, non-prescription medications, herbals, or other dietary supplements, you may benefit from medication therapy management services. People who may benefit the most include individuals who:

  • use several medications and/or dietary supplements
  • have several health conditions
  • have questions or problems with their medications
  • are taking medications that require close monitoring
  • have been hospitalized
  • see more than one doctor, OR
  • get their medications from more than one pharmacy.

Talking with your pharmacist regularly about your medicines will help you to stay healthier and spend less money overall on health care. Medication-related problems can be identified and resolved more quickly, and the information you gather will help you to communicate more effectively with your prescriber.

What can I expect from an MTM consultation?

Comprehensive Medication Review (CMR) 

CMR is our most popular service and includes:

  • Personal meeting with client (this may be done by telephone when necessary)
  • Development of complete medication profile (prescription and non-prescription medications, dietary supplements and vitamins, and herbal remedies)
  • Development and prioritization of medication-related problem list
  • Development of medication-related action plan
  • Communication with prescriber regarding medication-related problems
  • Follow up with client regarding prescriber decisions/changes
  • Follow up with client 2-4 weeks after changes are implemented

 

Other available services

Targeted Medication Review            

Includes:

  • Discussion with client regarding one specific medication-related problem
  • Communication with prescriber regarding specific medication-related problem
  • Follow up with client regarding prescriber decisions/changes
  • Follow up with client 2-4 weeks after changes implemented

 

Medication/Disease counseling and monitoring  

Includes:

  • Education about medication and/or disease
  • Follow up with client for a pre-determined period after initial consultation to optimize medication use.

 

Prescriber consultation (telephone/fax)     

Includes:

  • Communication with prescriber (written or spoken) on behalf of client
  • Written documentation (to both client and prescriber) of decisions/changes to be made
  • Follow up with client regarding decisions/changes
  • Follow up with client 2-4 weeks after changes implemented
